Insurance / Billing


We are a preferred provider for most plans provided by the following insurance companies:

Please call your insurance company to verify the benefits of your specific policy before your initial visit as you are ultimately responsible for payment. There are often deductibles, number of visit limits, and other restrictions. Coaching is not covered by insurance and coaching clients will need to abide by the payment practices for all sessions as described in the coaching payment agreement.

Being a preferred provider means that we are "in-network" . That means we will bill your insurance directly and you will be responsible for your co-pay (and deductible if applicable) at the time of your visit.


For insurance providers not listed above:

For insurance panels which Catherine Samson PMHNP-BC/Portland Family Wellness is an out-of-network provider, patients are responsible for payment-in-full at the time of service. We do recommend that you call the number on the back of your insurance card to educate yourself on the specifics of the mental health benefits/coverage of your insurance plan. We will provide you with a bill that you can submit to your insurance company. We do not submit bills to out-of-network insurance companies. It is the patients’ responsibility.

When you call in this situation ask the following questions:

  1. Is Catherine Samson, ARNP-BC a covered provider for me?

  2. What are my out-of-network benefits for mental health?

  3. Do I have a separate out-of-network deductible?

  4. What percentage or fee do I pay? Is this based on a usual and customary fee or her actual rates?

  5. How many visits do I get in a certain time frame?


Additional comments regarding Insurance:

Insurance no longer covers charges for services rendered by phone or e-mail, prior authorization calls or forms, letters, reports, cancellations (if less than 48 hours notice) or missed appointments.These will be billed to you directly. Payment is expected at the time of service. If you do not have a co-pay with you at the time of an appointment, a $35 charge will be added to your bill. Patients are charged $250 for a missed appointment and/or any appointment cancelled with under 48 hour notice.


*Clients with Blue Cross Blue Shield as their insurance provider should also call their insurance to determine if prior authorization is necessary.

** Providence members should call Providence Health Plan to check that we are within network prior their first visit to see us.
