If you are unable to download or print these forms please contact us to make arrangements to complete the paperwork.

• To be returned within 72 hours of scheduling your initial appointment:

  • Return via Spruce Health App, secured email, or fax to the practice at to (503) 713-5395 in order to confirm and hold your appointment time within 72 hours of scheduling the appointment. NOTE: All missed appointments or appointments cancelled with less than 48 hours notice, will result in a $250 to the client and the card on file.

• To be returned at least 48 hours before the first appointment:

  • Download these three additional forms, filling them out and return them via the secured Spruce Health App (invite will be sent to join when you are scheduled for your first appointment).
